DORA
The Colorado Department of Regulatory Agencies has responsibility
for regulating licensed and unlicensed psychotherapists in Colorado.
As an unlicensed psychotherapist in private practice, I have submitted
my practice information to DORA as required by law. The agency within
the department that has specific responsibility is found at DORA
in the Mental Health Section, 1560 Broadway, Suite 1370, Denver,
Colorado 80202, (303) 894-7766.
Therapy as a Process
Therapy is a process that requires considerable investment of time
and effort. Although each individual’s situation is unique,
there are common stages you can expect. Initially, we will work
to clarify your concerns and challenges. WE will explore your family
upbringing in light of your current concerns. Then, we will work
on changes and potential solutions that may work for you as you
face your difficulties. Finally, we will complete the process of
change and reflect on your courage, progress and growth.
Length of Treatment and Fees
At our initial visit, I will provide you with a comprehensive overview
of services available, office policies and treatment costs so that
you may enter into psychotherapy with fully informed consent. Treatment
sessions are 45 or 75 minutes in length generally one time per week.
The length or frequency of your case may be increased or decreased
based on your needs and goals. Your next treatment time will be
scheduled with you at the completion of each visit. Although emergency
situations may arise, I will do my best to be on time to reflect
my respect for you as an individual with your own time commitments
and responsibilities. A fee schedule is available for your review.
Copayments and coinsurance are expected at the time of service.
This office will file your insurance benefits if current and correct
information is given to us. You may be required to contact your
insurance company for benefit eligibility. Charges not covered by
your insurance remain your responsibility. You will receive a statement
each month around the 15th of the month following the prior month’s
charges.
Confidentiality
Generally speaking, information provided by and to a client during
a psychotherapy session is legally confidential if the therapist
is a licensed marriage and family therapist, a licensed social worker,
a licensed professional counselor, a licensed psychologist or as
an unlicensed psychotherapist. If the information conveyed is legally
confidential, the therapist can not be forced to disclose any information
without the client’s consent. There are exceptions to the
general rule of legal confidentiality. These exceptions are listed
in the Colorado Statutes (HB 1072, 12-43-214(1)(d)(iv). Please be
aware that except in the case of information given to a licensed
psychologist, legal confidentiality does not apply in a criminal
or delinquency proceeding. Other exceptions will be identified as
appropriate during the course of therapy.
Your Rights and Responsibilities
You are entitled to:
- Be treated with respect, dignity and in a professional manner.
- Ask me questions at any time regarding my methods of therapy,
therapeutic techniques, duration of therapy (if I can determine
it), and the fee schedule of this office.
- Seek a second opinion from another therapist or terminate therapy
at any time.
- A professional relationship where sexual intimacy between a
therapist and client is never appropriate. If sexual intimacy
occurs, it should be reported to DORA, Mental Health Section.
You are responsible for:
- Reviewing office policies and treatment approaches at the time
of your first appointment.
- Attending appointments as scheduled. Appointments broken (not
kept) without notice will be charged according to the fee schedule
provided to you at your first visit.
- Scheduling regular appointments if you are in agreement with
the plan of care recommended to you. You will be offered regularly
scheduled appointments according to your scheduling needs. I will
make every effort to accommodate specific appointment requests.
You may request additional appointments outside your regularly
scheduled time or alter your customary appointment times as permitted
by my schedule.
- Ask for a reminder call if you need help in remembering your
scheduled appointment times. If you need a reminder call prior
to an appointment, please ask for it. I do not routinely call
to remind of appointments unless asked. Please arrive on time
when you have a scheduled appointment.
- Cancel appointments with 24 hours notice. This notice will allow
me to offer your appointment time (for the canceled visit only)
to someone else in need. You will not be charged if you provide
me with 24 hours notice of any cancellation.
- Signing this disclosure statement and the informed consent paperwork
you received at the time of your first appointment.
Emergency Care
Although you will probably not need emergency services, please
go to your nearest emergency room for assistance. As a sole
practitioner in a small private practice, I do not provide 24-hour
coverage. Should you forget that you are to call the nearest emergency
room in an emergency situation, the voice mail will remind you.
